Majestic MD Membership Terms & Conditions

Please review the following Membership Terms & Conditions carefully. By enrolling in a membership or purchasing services or products from Majestic MD , you agree to be bound by these terms.

1. Membership Plans & Monthly Charges

By enrolling in a membership, you authorize Majestic MD to automatically charge the payment method on file on a recurring monthly basis until canceled in accordance with these Terms.

Membership Options

• Recharge & Restore Membership
  10 Credits per Month — $350/month (6 month minimum)

• Revitalize Membership
  15 Credits per Month — $450/month (6 month minimum)

• Ultimate Rejuvenation Membership
  25 Credits per Month — $625/month (6 month minimum)

• Platinum Vitality Membership
  50 Credits per Month — $1,000/month (6 month minimum)

2. Minimum Commitment Periods

• Memberships: Minimum 6-month commitment

• Product Memberships: Minimum 90-day commitment

Early cancellation penalties apply if the minimum commitment is not met (see Section 5).

3. Credits & Services

Memberships include monthly credits that may be applied toward eligible services.

Credit Examples (subject to change):

• Hyperbaric Oxygen Therapy (HBOT): 3 credits

• Infrared Sauna: 1 credit

• Float Therapy: 2 credits

• Compression Therapy: 1 credit

Credit Terms:

• Credits roll over for 90 days from the date issued

• Credits are non-transferable

• Unused credits expire after 90 days

• Credits have no cash value

4. Cancellations

After Minimum Commitment

Memberships may be canceled after the required minimum term by providing written notice at least 15 days prior to the next billing date. No penalty will apply if properly canceled.

Early Cancellation Penalties

• Memberships (less than 6 months):
  Penalty equal to one month of membership dues or the difference between the discounted membership rate and the retail value of services used (whichever is less).

• Product Memberships (less than 90 days):
  Penalty equal to the difference between the membership discount and full retail pricing.

5. Refund Policy – All Sales Final/No Refunds

All sales are final. Majestic MD does not offer refunds for any reason on the following:

• Services (including but not limited to injections, IV therapy, aesthetic treatments, wellness treatments, and medical services)

• Memberships (monthly or prepaid)

• Packages or bundles

• Retail products, including supplements, skincare, and medical or wellness products

This applies whether services or products are used in full, in part, or not at all.

Products

Due to health, safety, and regulatory standards, all product sales are final. No returns or refunds will be accepted on opened or unopened products.

Unused Credits

Unused credits do not convert to cash and are not refundable.

6. Store Credits (If Applicable)

At our sole discretion, Majestic MD may issue a store credit as a courtesy.

• Credits have no cash value

• Credits are non-refundable

• Credits may expire

• Acceptance of a credit constitutes full and final resolution of the transaction

7. Chargebacks & Payment Disputes

By completing a purchase or enrolling in a membership, you agree that initiating a chargeback or payment dispute for any transaction that is disclosed as non-refundable constitutes a violation of these Terms & Conditions.

If a chargeback or payment dispute is initiated:

• Any unused services, credits, packages, or membership benefits are immediately forfeited

• The client waives the right to receive any further services, including appointments or consultations with a medical provider, until the dispute is fully resolved

• Majestic MD reserves the right to suspend or terminate the client relationship

• The client remains financially responsible for any services already rendered

Chargebacks may also result in permanent account termination and denial of future services at Majestic MD .

8. Authorization & Billing

You authorize Majestic MD to charge your card on file for all applicable membership dues and fees until cancellation is properly completed in accordance with these Terms.

9. Termination by Majestic

Majestic MD reserves the right to suspend or terminate any membership for misuse, abuse of services, violation of policies, or nonpayment.

10. Medical Disclaimer & Liability Waiver

By participating in services, you represent and agree that:

• You have no medical condition that would prohibit participation, or you have consulted with and received clearance from your healthcare provider

• You are capable of participating without assistance (except minor assistance with HBOT)

• You understand that services may involve risks, including dizziness, fatigue, or skin sensitivity

• You voluntarily assume all such risks

• You release and hold harmless Majestic MD , its owners, employees, and contractors from liability, except in cases of gross negligence

The therapies offered—including but not limited to hyperbaric oxygen, red light therapy, sauna, cryotherapy, and IV nutrient therapy—are considered off-label uses and are not intended to diagnose, treat, cure, or prevent any disease.

11. Acknowledgment

By purchasing services, memberships, packages, or products from Majestic MD, you confirm that you have read, understood, and agreed to these Membership Terms & Conditions.
